La revue n’observe que le premier : le changement en source est l’événement visible, le remplacement de la copie installée ne l’est pas. La clôture est alors déclenchée par la correction de source et non par le comportement observé, ce qui est exactement la condition du failed fix : la vraie frontière était la distribution, pas la source.The correction had truly been written and approved in the book. Replacing the station card was another act that nobody followed. The kitchen could therefore keep a rule everyone believed removed.Installation copies rather than references, which splits the correction and its deployment into two separate events. Review observes only the first: the source change is the visible event, the replacement of the installed copy is not. Closure is then triggered by the source fix rather than by observed behavior, which is exactly the failed-fix condition: the real boundary was distribution, not the source.
